I had a Dodge D50 when I was younger. It had a Mitsubishi engine. The carburetor screws for the butterfly valve fell out into the engine. Somehow they made it to all four cylinders.

I had an Dodge 04' 5.9 H.O. Cummins ,¾ ton 4x4 quad cab, lost 2 pistons at 146k motor trashed, new long block installed. They said they couldn't find what caused it. Cost was $10k. I think they were lying about the actual cause of holes in two pistons.
